Good practice model for controlling pharmaceutical costs in hospitals (PDF)
Prepared by VicTAG for the Department of Human Services, the good practice model aims
to identify and evaluate management systems available to managers to monitor and control
non-wages costs in the key area of pharmaceuticals; and to identify those hospitals
that are operating at good practice levels in controlling these costs.
A package of five innovations to improve the management of pharmaceutical costs in Victorian
public hospitals has been proposed. These innovations include:
- a funding model for high cost pharmaceuticals
- the introduction of pharmacist formulary managers
- electronic prescribing with decision support tools
- state-wide cost minimisation activities, and
- improved reporting and data use.
Guideline for the use of oral methotrexate in hospitals (PDF)
Prepared by the Victorian Medication Safety Committee(VMSC) based on responses
to a questionnaire about current practices in Victorian hospitals. In addition
it draws from a NSW Health Department Information Bulletin No 93/31, the NHS Report:
Building a safer NHS for patients and an Institute for Safe Medication Practices (ISMP) Bulletin.

Prepared by VicTAG these recommendations aim to assist public hospitals implementing
the PBS for outpatient and discharge prescriptions.

Developed by VicTAG this pro forma can be used by the drug and therapeutics committees
of health care institutions when they consider applications for formulary additions.
The pro forma is available to be downloaded and modified to suit your hospital’s requirements.
